What the public site collects
The public inquiry and application forms are intentionally limited. They are designed to capture basic contact and fit information, not full medical records.
What the public site should not be used for
Do not send highly sensitive medical history, imaging, laboratory data, payment information, or emergency requests through the public website unless a later secure process is explicitly provided.
How submissions are handled
Public submissions are validated, routed to internal notification systems, and passed to downstream intake workflows configured by the site operator. Access is limited to team members who need the information for triage and response.
Future updates
This policy will expand as the secure intake flow, additional languages, and production infrastructure are finalized.
